CHRISTIAN FORMATION REGISTRATION FORMS
Christian Formation Registration is open! Register your child today and complete the Registration Form with the required documents: birth certificate and baptismal certificate for each child.
Confirmation students must also provide a First Communion certificate.

Payment Options: Online payment is available through the button below. Checks or money orders may be made payable to Saint Mary Catholic Church, with "Christian Formation" in the memo, and mailed to 7307 40th Ave., Kenosha, WI 53142, or dropped off at the back of the church or at the Parish Office.
